Your baby’s health organiser: 0-12 months
THE FIRST YEAR / Parent Library
Tests, check-ups, vaccinations and questions, with your own plan agreed with the clinic.
One organiser, several kinds of information
Your child’s health record remains the main health document. Our PDF has a different role: helping parents note dates, questions and things to check. Keep it with your documents or use a printed copy before appointments. You do not need to copy the whole medical history. What matters is distinguishing a completed test from one requiring further follow-up.
What should you check after discharge?
Ask which screening tests were performed, how they were recorded and whether any need repeating. Newborn tests include hearing screening, pulse oximetry and bloodspot screening. Arrange the midwife’s home visit and first clinic check-up too. A longer hospital stay or prematurity may require a different care arrangement.
Appointments during the first year
Poland’s NFZ identifies check-up periods in the first weeks, between the second and sixth month, and in the ninth and twelfth months. Your clinic sets the exact timing and scope. Prepare questions about feeding, development and behaviour. Do not delay reporting a worrying change until the next routine health review.
Why do we not provide one schedule for everyone?
The Polish PSO includes mandatory and recommended vaccinations. An individual plan also depends on the product selected and vaccination history, including the number of doses and intervals. The programme is updated every year. In the organiser, record dates confirmed by a doctor instead of treating a general file as a personal medical assessment.
Prepare questions before the appointment
Note current medicines, illnesses and previous vaccination reactions. Ask which symptoms are expected after the vaccine and when to seek help. Agree the next appointment before leaving the clinic. The organiser can then guide you from one concrete decision to the next, while supporting discussion with your care team.
Frequently asked questions
Does the organiser replace the child’s health record?
No. It helps you prepare questions and remember agreed dates. Healthcare professionals maintain medical information in the appropriate records. Do not copy information you do not understand into the organiser; ask for an explanation. Keep entries identifying the vaccine doses given in your child’s medical records.
What should I do after a vaccination is postponed?
Contact your clinic to agree the next steps. Do not calculate them yourself from a general calendar. Earlier doses, the vaccine product and age limits matter. Record the new appointment and the person you agreed it with. Bring the full records to the appointment if anything is unclear.
Is the plan complete after the first birthday?
The first year is only part of preventive care. At the last appointment covered by this organiser, ask about further checks and doses. Some services fall after the first birthday. Record the next step before closing the current checklist, so the age limit of our material does not become a gap in care.
How can I prepare important information for an appointment?
Write down specific observations, when they started and how they affect daily life. Bring lists of medicines, advice and records from other clinics. If something has changed, mention it at the start. Prepare three main questions and add the answers afterwards if helpful. Do not leave urgent problems until a routine health review.
How to use this material
Read the checklist, mark the items relevant to your circumstances and add your own arrangements. Follow individual advice for health matters. After a conversation, record the next step and date; return to the material as your needs change.
